●Assigns the Present on Admission (POA) indicator for each diagnosis after determining which conditions were present at the time of admission and which conditions developed during the course of the hospital stay.
●Verifies each record that patient status, admitting/attending physician, admission date and time, discharge date and time, death date and time is accurate before coding and finalizing the chart.
●Queries providers when documentation is determined to be insufficient, conflicting, or ambiguous to elicit documentation reflecting the most accurate and specific conditions or procedures. Diplomacy and tact is exercised when dealing with all physicians.
●Works with the Clinical Documentation Specialists regarding coding and Diagnosis Related Group (DRG) assignment based on clinical indications documented in the chart.
●Maintains knowledge and skills via written coding resources, clinical information, webinars, and monthly coding meetings
